Flapless Surgery

Also known as: Flapless implant placement, minimally invasive implant, punch technique

Implant placement without making a full incision — a minimally invasive approach.

Flapless surgery refers to implant placement without raising a full mucoperiosteal flap (cutting and peeling back the gum). Instead, a small circular punch of gum tissue is removed, or the implant is placed through the gum using a guided surgical template.

Advantages

  • Less surgical trauma and post-operative discomfort
  • No sutures required
  • Faster healing
  • Less bleeding
  • Shorter surgical time

Limitations

  • The surgeon cannot directly visualise the bone — requires very accurate pre-surgical planning (CBCT + guided surgery)
  • Not suitable for sites requiring simultaneous bone grafting
  • Risk of thermal damage if the drill deviates from planned path

When it is used

Flapless surgery is most appropriate for single implants in sites with ample bone volume, confirmed by CBCT scanning, using a surgical guide. It is commonly used in guided surgery protocols where digital planning allows very predictable outcomes.
