Night Guard

A custom-made appliance worn at night to protect implants from grinding forces.
A night guard (occlusal splint) is a custom-made appliance, usually of hard acrylic, worn over the teeth during sleep to protect them from the damaging effects of grinding and clenching (bruxism).
Why it matters for implants
Implants lack the shock-absorbing periodontal ligament of natural teeth. Grinding forces — which can be three to ten times greater than normal chewing forces — are transmitted directly through the crown and abutment to the implant and bone. Without protection, these forces can fracture ceramic components, loosen screws, or accelerate bone loss around the implant.
Types
- Hard acrylic splint (Michigan splint): The gold standard for bruxism. Covers the upper teeth, allows the lower teeth to glide smoothly without damaging contact. Distributes forces evenly.
- Soft splint: More comfortable initially but may actually increase clenching activity in some patients. Generally not recommended for heavy bruxists.
- NTI device: Covers only the front teeth, reducing masseter muscle activation. Useful for jaw pain but not appropriate for full bite protection of posterior implants.
Cost and lifespan
A custom night guard typically costs £150–£300. With proper care (daily cleaning, storage in a case) it may last 5–10 years, though heavy grinders may wear through one faster.
